Cyst Removal and Abscess Treatment at Pandora Family Medicine

The two most common things we see are sebaceous (skin) cysts and abscesses. A sebaceous cyst is a slow-growing, closed sac under the skin, usually painless unless it gets infected. An abscess is a pocket of infection that tends to come on faster, feels warm and tender, and often needs to be drained sooner rather than later.

We also treat epidermoid cysts, lipomas, and pilonidal cysts, as well as infected ingrown hairs that have developed into a localized abscess. If you are not sure what you are dealing with, that is fine. Describe what you are noticing when you call or book, and we will let you know whether an in-office visit makes sense.

Most of these do not require a referral to a dermatologist or surgeon. A family physician can handle the large majority of cysts and abscesses in the office, which means faster care and lower cost for you.

   Before the procedure

   Dr. Gougian will examine the area first. She will confirm what type of cyst or abscess it is, check for signs of spreading infection, and talk through what she is going to do. You will have a chance to ask questions before anything happens.

   Local anesthetic

   The area is numbed with a local anesthetic injection before anything else. The injection itself stings briefly, similar to what you feel at the dentist. Once the area is numb, you should feel pressure but not pain during the procedure.

   Incision and drainage or excision

   For an abscess, Dr. Gougian makes a small incision to drain the infection and clears out the pocket. For a cyst, the approach depends on whether it is infected and how deep it sits. An inflamed cyst may be drained first to reduce swelling; a stable cyst can often be excised in one visit, meaning the sac is removed completely. Removing the sac matters because it reduces the chance the cyst comes back.

   After the procedure

   Most procedures take between 15 and 30 minutes. The wound may be packed with gauze if it needs to drain over a day or two, or it may be closed with a small bandage, depending on the situation. Dr. Gougian will walk you through exactly what the wound will look like, how to care for it at home, and when to come back if needed. You leave with written aftercare instructions.

   A note on same-day access

   Abscesses in particular should not wait. If you are in pain or the area is growing, call us at 301-200-0960. We make room for same-day visits when something needs prompt attention. That is different from urgent care, where you are one of dozens of walk-ins. Here, Dr. Gougian is your doctor, and she will know your history when you walk in.

We know the timeline matters. Many patients are working against an interview date or a request for evidence, and we schedule and process exams with that in mind. The exam is a flat fee quoted before you book; blood tests and vaccines are itemized and added only if you need them, so there are no surprise costs.

Some cysts sit quietly for years and never cause problems. Others get infected, grow quickly, or become painful enough to affect daily life. You should come in if the area is red, warm, or swollen; if it is growing noticeably; if you have a fever; or if it is draining on its own. Any of those signs means the body is dealing with active infection, and waiting usually makes things worse.

You do not need to be in crisis to come in. If a cyst has been bothering you, or you just want it gone, that is a completely reasonable reason to schedule a visit.

When in doubt, call. We can often tell you over the phone whether something sounds like it needs same-day attention or can wait for a scheduled appointment.

Most patients go home the same day and return to normal activity within a day or two, depending on the size and location of the wound. If the wound was packed, you will come back in 24 to 48 hours to have the packing removed and the site checked. Dr. Gougian will tell you what to watch for at home: increased redness, warmth, or discharge that suggests the infection is not clearing.
